What is PC-IC in LTC?
Person-centred integrated care prioritises the holistic needs of individuals, bridging gaps between health and long term care needs as well as social needs. This approach forms the foundation for ecosystems that enhance collaboration, improve outcomes, and ensure sustainability in LTC settings.
The Study
Researchers conducted a scoping review between May and August 2024, analysing 1,862 publications and selecting 37 for detailed examination. Using a Population-Concept-Context (PCC) framework and databases such as Medline and Scopus, the study explored the contextual factors critical to PC-IC ecosystems based on the lens of internal relationships and framing influences on a care triad

The findings underscore the need for EU countries to strengthen their PC-IC frameworks, as they currently trail behind other OECD nations in these areas.
The Way Forward
Aligning regulatory policies with effective PC-IC models and enhancing collaboration and infrastructure are essential next steps. This research provides the groundwork for developing a typology aligned with existing OECD LTC frameworks, enabling scalable and effective care solutions across Europe.
